Context

What forces are at play: the problem, the constraints (technical, licensing, people), the options that were considered. Neutral, factual, short.

Decision

What we decided, in active voice ("cheme-as-code does X"). Name the rule precisely enough that a reviewer can tell whether a change follows it.

Consequences

What becomes easier and what becomes harder. List both:

• Positive: ...
• Negative: ...
• Known limitations and open follow-ups (link the issues).

Evidence

• PR or commit that introduced the decision (#NNN, short hash).
• Files that implement or enforce it (code, tests, docs).
